The following section is designed to help you understand the application and provide additional information regarding some of the questions. Please use it as a quick reference as you complete the application. The number(s) in parentheses indicates the application question(s) explained in the passage. All of this information is used, upon enrollment, as part of your permanent student record. When you are ready to apply, click on the link to download the application.
Section 1
(1) Sometimes students have had a different last name at some point in their lives. UT needs this information when processing admission materials, such as transcripts, in order to match them to the name(s) on the application.
(2) UT uses your Social Security Number as an identification and tracking number. Once you enroll, it becomes your permanent student ID number.
(8) If you are not a citizen of the United States but are a permanent resident, you must submit a copy of your alien registration card (green card), front and back, and indicate your resident number and country of citizenship. If you are not a U.S. citizen or permanent resident, you need to apply through International Admission, Rocket Hall Room 1300. You can contact this office at 419.530.8888.
Section 2
(11-14) Residents of Ohio for the 12 months immediately preceding enrollment are eligible to pay in-state tuition and fees. In addition, any residents of Monroe County, Michigan for the 12 months immediately preceding enrollment are eligible to pay Ohio-resident tuition rates. These questions must be answered in order to process your application.
The following situations would also qualify a student not meeting the above statement to be considered in-state. However, special documentation is necessary and can be sent at the time of application. Note: All references below to Ohio are interchangeable with Monroe County, Michigan.
Rule C-1: He or she is the dependent of a parent who has resided in Ohio for the past 12 months and has paid Ohio income tax.

Rule C-3: He or she is the dependent of a parent or legal guardian or is the spouse of someone who accepted full-time employment in Ohio and established domicile in Ohio as of the first day of the term.

Exception E-1: He or she is full- or part-time employed and is self supporting on Ohio taxable funds from employment (not including additional aid or source of income that may be received) and will be attending on a part-time basis through the 12-month period.

Exception E-2: A person who enters and remains on active duty status in the U.S. military service while a resident of Ohio for all other legal purposes and his/her dependents shall be considered Ohio residents, as long as Ohio remains the state of such person's domicile.

Exception E-3: A person who is on active duty status in the U.S. military service and stationed and resides in Ohio and his or her dependents shall be considered residents of Ohio.

Section 3
(15) Be sure to review the academic requirements of your preferred major. Select from the list of majors and write your three to five character code in the boxes, using one character per box. If you are unsure about your major but have a preferred college, use the undecided code within that college. If you are unsure about your major and college, use the Quest code.
Section 4
(19-22) Students who have taken college course work while in high school through Ohio's post-secondary enrollment options program or other concurrent enrollment programs must indicate this in question 19 and must also indicate the college(s) attended, dates of attendance and credit attempted in questions 20 and 21. Transfer students must also fill out questions 20 and 21. All regionally accredited colleges/universities attended must be listed. If you have been suspended or dismissed from another college or university, this must be indicated in question 22.
