Utilize this packet if you'd like to start a new student organization at UT.
Officer Listing Form
This form must be completed within the first six (6) weeks of each semester in order to retain active status for your organization.
Student Organization Office Space Application
Applications for Student Organization Office Space for the 2008-2009 academic year are due no later than April 30, 2008.
PLANNING A STUDENT ORGANIZATION SPONSORED EVENT
Event Request Form
Complete this form for EVERY event sponsored by your organization. Prior to submitting this form, be sure to call and place a hold on your requested venue. Your room reservation will be confirmed after this form has been fully approved.
Academic Room Request
Complete this form for events/meeting sponsored in Academic buildings or classrooms. Use of this form does not require the completion of the Event Request Form. Prior to submitting your request for an academic room, CLICK HERE to submit an electronic request for your desired space. Once you have comfirmed the space's availability, complete this form and submit it to the Office of Dean of Students within 72 hours. Your room reservation will only be confirmed once the form has been received.
Outdoor Area/Equipment Request
Complete this form for all events and/or activities taking place outdoors. This includes but is not limited to sidewalk chalking, yard signs, outdoor picnics, concerts, etc.
Social Event Policy & Form
Events such as dances, concerts, ice breakers and parties constitute Social Events, and must complete this form in accoradance with the attached policy.
Donation Form
Student Organizations wishing to request donations of any kind from commercial vendors or other sources, including AVI FoodSystems, must complete this form.
Fundraising/Solicitation Form
If your event is a fundraiser, you must complete this form.
Party Pack Order Form

Application for Use of Informational Kiosks
There are eleven (11) kiosks on campus that you can utilize to promote your organizational events.
Student Union Large Publicity Request
Use this form to request a banner be hung in the SU, or reserve the display case or covered bulletin board for your event.
Student Union Shadowbox Application
There are 70 shadowboxes available in the South Lounge for Student Organizations to advertise their organization, events, meetings, etc. Shadowboxes are issued on a first come first serve basis, so request yours today!
Directory Information Request
Have a target audience in mind? This form must be utilized, and you must work closely with the Office of Dean of Students, to target specific groups of students on campus.
STUDENT ORGANIZATION TRAVEL
Travel Authorization Form
Complete this form at least 2 week prior to your Student Organiztaion sponsored trip. All travelers must also sign the Liability Release Form prior to departure.
RISK MANAGEMENT
Risk Assessment Form
Use this form to assess the risks associated with your event.
Alcohol Registration Form
All events where alcohol will be involved must be registered using this form.
Liability Release Form
This form should be utilized at events where there is potential risk involved in an activity taking place.
